My intake paperwork is uploaded on my secure client portal at https://melissa-grasso.clientsecure.me.  Once we schedule our first appointment, I will invite you to create a client portal account via email.  On the portal, you will be able to complete and sign your paperwork, upload documents, and access billing information.
